A winner was selected from 24 designs submitted by 10 freelance designers.
Can a printable interview/questionnaire page be eye-catching? We're confident that it can if great designers get their hands on it! We need a quick turnaround on a design to make this page come to life!

We are building a database-driven interview web page that will be printed and used as a tool for in-person interviews. Note that the page needs to be printable - so we can't make use of tabs and such...everything needs to be displayed on the page. The page will need to display up to 200 questions, so it's expected that the user will need to scroll up and down to view the page before it is printed. Each question will have an answer area that can be one of the following:
* text area
* checkboxes
* picklist
* radio button
The questions will be grouped into top level category groupings and then into questionnaires within the category grouping:
